Patient Surveys and Questionnaires
City Health Care Partnership CIC has now completed it's 2011/12 Annual Patient Experience Questionnaire
SMSR Ltd was commissioned to carry out the survey on behalf of CHCP and a total of 3,275 questionnaires were distributed across the service areas, either by post or hand distribution where this was not possible. The coverage of the survey encompassed 31 discreet areas of delivery across both Hull and the East Yorkshire.
