We understand that patients can suffer illness or require nursing support outside of normal working hours. Therefore, the service we provide spans the time when other services are closed. The Out of Hours Nursing Service is a continuation of the daytime District Nursing Service, which provides a wide range of nursing care to patients in Hull.
Staff will work in pairs (one Qualified Nurse and one Health Care Assistant) to visit patients within their homes in order to deliver nursing care.
Planned care – visits are requested by health care professionals to deliver nursing care during 5pm – 7am. This care usually comprises of care that requires more than one or two visits a day such as wound care, injections or eye drops.
Unplanned/unscheduled care – throughout the evening and night time referrals can be made from patients/carers and other out of hours care providers for nursing care and advice due to a ‘crisis’ such as symptom control, catheter care, assessment for changes in clinical condition or verification of expected death.
Patients can be referred to the service if they are 18 yrs or older and are registered with a Hull GP.
The service operates from ?5pm until 7am seven days a week.
There are two shifts:
Evening shift - 17.00hrs - 23.00hrs
Night shift - 21.30hrs - 07.00hrs
