Community Heart Failure Nursing Service
The Community Heart Failure Nursing Service is a team of nurses who have specialist knowledge of heart failure. They provide support and education to patients with heart failure within a clinical setting or in an individual’s home.
The service also includes a Heart Failure Rehabilitation Programme, following assessment, which is held twice a week at the Marfleet Primary Care Centre, Preston Road, Hull.
This programme is multi-agency and includes:
- Physiotherapy
- Dietary advice
- Medicine management advice
- Occupational therapy.
The service is also developing a support group for patients with heart failure and their carers.
The Community Heart Failure Nursing Team sees adults over the age of 18 years who are registered with a Hull GP.
The service is available Monday – Friday 08.30am – 5pm.
